Problem with Extension Tube

I recently purchased a used 21mm extension that appears in good condition, but the slot in the drive shaft does not line up with the red dot, so I cannot mount it on the camera. I have tried turning the slot with screwdriver, both clockwise and counterclockwise. I do not want to force it, but it does not budge. Is there some secret to aligning the slot with the dot?

Sadly, no. There is no secret. If attempts to turn the slotted end clockwise do not lead to a result, the extension tube will need seeing to.
Tubes are not very complicated things. But it will perhaps be cheaper to find another used one than have this one repaired.
